POSITION OVERVIEW
Harbor Health is seeking an experienced Integrated Care RN to serve as a leader in member Care Management. In this role, the nurse will guide members across Chronic Care Pathways and Complex Care Pathways, optimizing care coordination and improving patient outcomes. This position is vital to Harbor Health's mission to provide innovative, member-centered care while enhancing overall quality and efficiency.
POSITION DUTIES & RESPONSIBILITIES:
- Lead care coordination efforts for members navigating chronic and complex care pathways.
- Perform comprehensive clinical assessments to identify member needs and design individualized care plans.
- Serve as a point of contact for members, families, and the care team to ensure seamless communication and coordination.
- Monitor treatment plans, assess outcomes, and adjust care pathways as needed for member success.
- Collaborate closely with Clinicians, Specialists, Physician Assistants, Nurse Practitioners, and other healthcare professionals to optimize care delivery.
- Facilitate timely follow-up appointments, tests, and therapies while ensuring thorough electronic documentation.
- Proactively address barriers to care and connect members with community resources as appropriate.
- Demonstrate clinical expertise in managing patients with complex medical conditions across the care continuum.
- Provide member education to enhance understanding of care plans, treatments, and self-management strategies.
- Foster a member-centric culture that prioritizes safety, dignity, and respect in all interactions.
DESIRED PROFESSIONAL SKILLS & EXPERIENCE:
- Current RN license required
- BSN preferred but not required
- ER or ICU experience preferred but not required
- Minimum 2 years of nursing experience with a strong preference for experience in complex care or care coordination.
- Experience in care management, chronic disease management, or transitional care is a plus.
- Proficiency with electronic health records (EHR) and care coordination platforms.
- Strong critical thinking and problem-solving skills to navigate complex patient needs.
- Demonstrated ability to work independently while collaborating effectively in a team-based model.
- Excellence in communication and member education to ensure high levels of engagement and adherence.
- BLS certification required.
- Physical ability to perform required duties, including lifting and repetitive movements.
