POSITION OVERVIEW
We’re looking for an LVN  to operate within our Call Center who is passionate about delivering high-quality care through advanced clinical support and care coordination. In this role, you’ll address intermediate and complex member needs while ensuring seamless transitions in care remotely.

POSITION DUTIES & RESPONSIBILITIES

Advanced Clinical Needs:

  • Manage medication refill requests, ensuring accuracy and timely processing.
  • Address complex abnormal lab results, providing clear instructions, follow-up steps, and escalation when necessary.
  • Handle complex member questions, using clinical expertise to provide actionable solutions.

Specialist Communication and Care Coordination:

  • Respond to calls from specialists, acting as a liaison between members, providers, and the care team to ensure continuity of care.
  • Facilitate care coordination for members with chronic conditions or those requiring multi-specialty care.

Member Support and Collaboration:

  • Provide real-time support for member concerns, leveraging your nursing knowledge to deliver clear and empathetic guidance.
  • Collaborate with CMAs, RNs, and other team members to ensure high-quality care outcomes.
  • Document all clinical interactions and care plans in the EHR system with precision.
 

DESIRED PROFESSIONAL SKILLS & EXPERIENCE

  • Licensed Vocational Nurse (LVN) with active state licensure.
  • 2+ years of clinical experience; prior experience in a healthcare call center or telehealth preferred.
  • Comfort and experience working fully remote.
  • Strong understanding of clinical workflows, medical terminology, and nursing protocols.
  • Proficiency with EHR systems and triage tools.
  • Excellent critical thinking, communication, and interpersonal skills.
  • This is a fully remote role
