POSITION OVERVIEW
The Physical Therapist at Harbor Health collaborates with our interdisciplinary healthcare team to provide high-quality, personalized physical therapy services to our members. This role focuses on evaluating, planning, and implementing treatment programs that improve mobility, reduce pain, and enhance the overall quality of life for our members. As a Physical Therapist at Harbor Health, you will contribute directly to our mission of transforming healthcare in Austin through a member-centered approach that integrates comprehensive care with innovative payment models.

POSITION DUTIES & RESPONSIBILITIES

  • Conduct comprehensive initial assessments of members to develop individualized treatment plans
  • Design and implement evidence-based physical therapy interventions tailored to each member's unique needs and goals
  • Utilize manual therapy techniques, therapeutic exercise, and functional training to improve members' mobility and function
  • Document member progress accurately and maintain detailed electronic health records
  • Collaborate with interdisciplinary team members to ensure coordinated care delivery
  • Develop home exercise programs and provide education to members and their families
  • Participate in care planning meetings to discuss member progress and adjust treatment strategies as needed
  • Stay current with best practices in physical therapy and implement new evidence-based approaches
  • Monitor treatment outcomes and adjust plans to optimize member recovery and function
  • Maintain compliance with all relevant regulatory requirements and organizational policies
 

DESIRED PROFESSIONAL SKILLS & EXPERIENCE

  • Doctor of Physical Therapy (DPT) degree from an accredited institution is preferred, not required
  • Current state licensure as a Physical Therapist in Texas
  • 3+ years of clinical experience, preferably in an outpatient or integrated care setting
  • Strong understanding of musculoskeletal anatomy, biomechanics, and therapeutic interventions
  • Experience with electronic health record systems and documentation requirements
  • Excellent interpersonal skills with the ability to build rapport with diverse member populations
  • Demonstrated ability to work collaboratively as part of an interdisciplinary healthcare team
  • Strong problem-solving and clinical reasoning skills
  • Commitment to ongoing professional development and evidence-based practice
  • Experience in value-based care models preferred but not required
