The ACO Provider Relations Manager plays a crucial role in improving healthcare delivery and access by fostering provider relationships and expanding the ACO network. This position leads initiatives to enhance value-based payment models, drives provider engagement, and supports stakeholders and community partners to optimize processes and outcomes.

POSITION DUTIES & RESPONSIBILITIES

  • Conduct in-person visits to promote best practices adoption and technology utilization among participating providers
  • Collaborate with Business Development leadership to identify and pursue ACO growth opportunities
  • Identify performance improvement opportunities, establish collaborative goals, and monitor outcomes
  • Build and maintain provider relationships while providing data-driven insights on preferences and improvement opportunities
  • Support organizational and clinical leaders in goal alignment and achievement
  • Develop and execute strategic service line and outreach initiatives
  • Create quarterly strategic targeting objectives aligned with market strategies
  • Collect and analyze provider feedback to enhance Harbor's services and technology support
  • Gather and interpret relevant data to inform decision-making

DESIRED PROFESSIONAL SKILLS & EXPERIENCE

  • 5 years of healthcare experience
  • Conduct in-person visits to promote best practices adoption and technology utilization among participating providers
