POSITION OVERVIEW
The ACO Provider Success Consultant serves as a key strategic partner in Harbor Health's mission to transform healthcare delivery. This role bridges the gap between traditional healthcare models and value-based care by guiding providers through the ACO REACH framework. The consultant will drive provider engagement, optimize performance metrics, and ensure successful implementation of value-based care initiatives while maintaining focus on superior member outcomes.
POSITION DUTIES & RESPONSIBILITIES
- Build and nurture strategic provider relationships by serving as their primary point of contact for ACO REACH program support and value-based care guidance
- Design and implement customized practice transformation strategies that align with both provider capabilities and Harbor Health's objectives
- Analyze performance metrics and translate complex data into actionable insights that drive provider success in value-based care arrangements
- Lead regular provider meetings to review performance, identify improvement opportunities, and develop action plans to achieve quality and cost targets
- Collaborate with cross-functional teams to ensure providers have access to necessary resources, tools, and support systems
- Monitor and support provider compliance with ACO REACH requirements while identifying opportunities for program optimization
- Drive member attribution growth through provider engagement and strategic practice expansion initiatives
DESIRED PROFESSIONAL SKILLS & EXPERIENCE
- 5+ years of healthcare experience with demonstrated expertise in value-based care models, particularly ACO REACH or similar alternative payment models
- Proven track record of building and maintaining strong provider relationships in a healthcare setting
- Strong analytical capabilities with experience in healthcare data analysis and performance improvement methodologies
- Experience in practice transformation and change management within healthcare organizations
- Superior project management skills with ability to manage multiple provider relationships simultaneously
- Deep understanding of healthcare quality metrics, risk adjustment, and population health management
- Exceptional communication skills with ability to influence and engage providers at all levels
- Bachelor's degree required; Master's degree in healthcare administration, business, or related field preferred
- Willing to travel occasionally to the New York and Dallas Forth Worth areas
- This is a remote role with travel expectations
