POSITION OVERVIEW
Harbor Health is seeking Care Coordinators to join our clinic operations team in South Austin, TX. Harbor Health is an entirely new multi-specialty clinic group in Austin, TX utilizing a modern approach to co-create health with those who get, give, and pay for it — allowing everyone to fully flourish. In this pivotal role, you will efficiently manage and organize incoming clinical communications, ensure timely responses, and facilitate effective communication between providers and members. You will work closely with our clinic care team to manage clinical inboxes and address member questions and concerns, bringing strong organizational skills, attention to detail, and a thorough understanding of clinical terminology to everything you do — in alignment with our mission of transforming healthcare through our payvider model.
POSITION DUTIES & RESPONSIBILITIES
- Monitor and prioritize incoming clinical communications, including member cases and portal messages, based on urgency, member priority, and clinical relevance
- Route communications to the appropriate clinician or clinic staff for timely review and response; escalate urgent messages to the appropriate personnel for immediate attention
- Serve as a liaison between providers, members, and other stakeholders; facilitate communication between clinicians regarding care plans, test results, and treatment updates
- Maintain accurate records of all incoming and outgoing communications; document member inquiries, responses, and follow-up actions in the EHR or other designated systems
- Review outgoing communications for accuracy, completeness, and adherence to clinical guidelines
- Identify opportunities for process improvement and efficiency enhancement in clinical communication workflows
- Collaborate with healthcare professionals to develop and implement best practices for inbox management
- Provide training and support to clinical staff on effective inbox management techniques and tools
- Offer guidance on navigating electronic communication platforms and EHR systems
- Address inquiries and troubleshoot issues related to inbox management as needed
- Participate in special initiatives at the direction of market leaders
DESIRED PROFESSIONAL SKILLS & EXPERIENCE
Required:
- 3+ years of experience in a medical office or healthcare setting
- 2+ years of experience working as a Medical Assistant
- Strong organizational skills, attention to detail, and a thorough understanding of clinical terminology and procedures
- Comfort with technology, including the Microsoft suite of products
- Prior experience using electronic health records, including data capture, data mining, and reporting
- Ability to manage multiple tasks simultaneously and maintain an organized, systematic workflow
- Flexibility in an ambiguous and dynamic environment with a positive, solutions-oriented attitude
- Demonstrated ability to engage effectively and with cultural sensitivity across diverse populations
- Strong problem-solving skills and sound judgment for identifying and escalating issues as needed
- Low ego and humility; ability to build trust through clear, consistent communication
Preferred:
- Current Medical Assistant Certification
- Bilingual in English/Spanish
