Care Coordinator - Sandy Creek
POSITION OVERVIEW
Harbor Health is seeking a dedicated Care Coordinator for our Village Medical Clinic at Sandy Creek in El Paso to serve as a vital link between our members and their comprehensive care teams. In this role, you will facilitate member engagement, coordinate care plans, and ensure seamless transitions across all touchpoints in our integrated payvider model. The ideal candidate is passionate about member-centered care and thrives in a collaborative environment where they can make a direct impact on health outcomes for underserved communities in Austin.
POSITION DUTIES & RESPONSIBILITIES
- Connect with members to facilitate enrollment in clinical services and programs, driving awareness of available health services and resources
- Collaborate with physicians, nurses, and care team members to ensure members receive coordinated, comprehensive care across multiple touchpoints
- Provide member education on health conditions, medications, treatment plans, and care instructions to members and their families
- Monitor members during care transitions, including hospital discharges and skilled nursing facility stays, to ensure continuity of care
- Accurately document member interactions and maintain compliance with all processes related to member engagement and care coordination
- Build and maintain trusted relationships with members, providers, and the broader care team
- Provide administrative support including scheduling appointments, coordinating with external vendors, and managing insurance-related tasks
- Utilize multiple healthcare systems simultaneously to track members throughout their care journey and maintain accurate records
DESIRED PROFESSIONAL SKILLS & EXPERIENCE
Required:
- 2-3+ years of experience in a medical office or healthcare setting
- Strong communication skills with the ability to explain complex health information clearly to diverse populations
- Excellent organizational skills and attention to detail
- Ability to manage multiple priorities and work effectively in a fast-paced environment
- Proficiency with healthcare technology platforms and electronic health records
- Commitment to member-centered, value-based care principles
Preferred:
- Knowledge of medical terminology
- Certified Medical Assistant (CMA) or Community Health Worker (CHW) certification
- Experience working in underserved communities or with diverse populations
- Familiarity with value-based care models or integrated delivery systems
- Bilingual skills (English/Spanish)
